Delta Health Partners Initiative: Patients Advancing Their Health (PATH)

A grant to sustain and expand the PATH project, which integrates patient-centered Community Health Workers (CHWs) into complex care management teams in primary care medical practices to improve patient outcomes and lower medical expenditures for high-need/high cost Medicaid and Dual Eligible beneficiaries living in the Delta region of Arkansas.
